A case of Gestational Diabetes Mellitus with hour glass uterus.

Gestational diabetes is a condition diagnosed during pregnancy in a female with no prior history of diabetes. Although, these females are at a risk of developing Type2 Diabetes in later life. So this is a case of a very difficult/ High risk pregnancy & Delivery.
A primigravida (first time pregnant) patient was diagnosed with High Blood sugar level at 24weeks of gestation at our hospital as we follow a strict screening protocols for all our ANC patients. It is really common yet if not diagnosed in time then might have serious complications on both the mother n baby as well.
This patient was admitted with 38.4 weeks pregnancy with normal body temperature, pulse: 94beats/min and B.P: 130/90mmhg

Cervical dilation started normally and progressed upto full dilatation very smoothly after the spontaneous rupture of membranes but as She was having hour glass (irregular) contractions, the head of the baby was not coming down. We started with utero-tonic agents and monitoring blood sugar levels every hour with fetal heart rate monitoring. Finally the head decent down and she delivered normally through vagina. Baby cried immediately after birth. Cord clamped n cut and the baby was handed over to paediatrician.
There was no spontaneous separation of placenta due to irregular contractions, Hour glass uterus, where in the placenta got trapped inside the uterus. So manual removal of placenta was done by Dr. Kaajal Mangukiya at our OORKID Hospital And IVF Center.

Even after delivery, we kept a close watch on the blood sugar levels of the month and the baby. Both mother and the baby are fine now and discharged after two days.

Note: In india, Approximately around 20% incidences of Gestational Diabetes occurs and that’s just the tip of the iceberg as many females in rural area suffer from the complications because of lack of awareness.
